A remediation portfolio, built section by section

The Remediation Evidence Portfolio Builder helps you put together a remediation portfolio for a fitness to practise concern in one organised place. It is a tool inside our student portal. You work through nine sections, from a short summary of the concern to an action plan with dates, and the builder turns your entries into a tidy, numbered document with a cover page, contents and an evidence index that you can save as a PDF.

It does not write anything for you, and it does not judge your case. What it does is give structure to the evidence you are already gathering: courses and certificates, reflections, testimonials, changes to your practice and CPD. Courses you complete on our portal appear in the portfolio automatically, with their completion dates and certificate numbers, so you are not copying details by hand.

Access lasts 12 months. See the price table on this page for the current price, including the option to buy the builder together with the Reflective Practice Workbook.

Who the portfolio builder helps

The builder is for registered healthcare professionals in the UK who need to show what they have done to address a concern, and who want that evidence to be clear and easy to follow. It is most useful if you are:

  • at an early stage, after a concern has been raised, and want to start organising evidence from the beginning rather than in a rush later;
  • under investigation, gathering material your adviser may share with case examiners or an investigating committee;
  • working under an interim order or conditions, and need a running record of what you have done to meet them;
  • preparing for a hearing or a review hearing, where a panel will want to see a coherent account of your insight and remediation;
  • returning to practice or applying for restoration, and need to show what you have done since;
  • keeping a development record with no current case, perhaps after a complaint or a learning event at work.

These stages appear as options on the portfolio cover, so the document states clearly where you are in the process. The builder is also useful to defence advisers and supervisors, who can review a single well-ordered document rather than a folder of separate files, but the account and its contents belong to you.

What is inside: the nine portfolio sections

The builder has a section navigator down the side, and each section has its own fields. A completeness meter shows how far you have got and which sections still need work.

  1. Cover. Your name, registration number (optional), regulator, the stage you are at and the date prepared.
  2. Concern summary. A short, anonymised summary of the concern in your own words. The meter looks for at least 60 words, enough to set out what happened without detail that could identify anyone.
  3. Chronology. Dated entries for what happened and what you have done since, with optional notes. A clear timeline helps a reader see the sequence of your learning. The free hearing timeline planner is a useful companion for planning ahead.
  4. Courses and certificates. Courses you complete on our portal are listed for you, with completion date, hours and certificate number; you can untick any that are not relevant. You can also add other learning by hand, with the provider, date, hours and evidence reference.
  5. Reflections. Dated reflections with a title. You can paste them in, or, if you also have the Reflective Practice Workbook, import reflections you wrote there.
  6. Testimonials. A tracker for each referee: name or initials, role, date requested, date received and status (not yet requested, requested, received or declined). Our free testimonial request generator helps you write the request itself.
  7. Practice changes and audits. What you changed, when, and the evidence, such as an audit result or a revised checklist.
  8. CPD log. Dated CPD activities with hours and key learning.
  9. Action plan. Actions with a target date and a tick when done, so the portfolio looks forward as well as back.

The completeness meter: what it checks

The meter gives a simple score out of 100 based on clear rules, so you can see gaps at a glance. It checks, for example, that the cover has your name, regulator and stage; that the concern summary has at least 60 words; that there are at least three dated chronology entries; at least two relevant courses; at least two reflections of 100 words or more; at least two testimonials requested and one received; at least one practice change with evidence; at least three CPD entries; and at least three actions with target dates.

These thresholds are prompts, not standards set by any regulator. A full meter means the portfolio has something in every section; it does not mean the evidence is strong enough for your case. Only you and your advisers can judge that, and regulators decide each case on its own facts.

Exporting your portfolio

When you are ready, the builder produces a print-ready document: a cover page, a contents list, numbered sections and an evidence index that lists each certificate, reflection, testimonial and practice change with the section it sits in. Use your browser's print option to save it as a PDF. You can also download a plain-text version, which is handy if your adviser wants to paste sections into their own documents.

Export as often as you like. Many people export a version before each meeting with their adviser, so there is a dated record of how the portfolio developed.

How it differs from the free tools

Our free tools and the remediation pathway remain free, and many people need nothing more. They run in your browser, store your entries only on that device and each one handles a single task: check your readiness, plan a timeline, draft a reflective statement or a testimonial request, then print or save.

A sensible route is to start with the free readiness check, then move to the builder once you are gathering evidence over weeks or months.

Privacy: your data, your account

Your portfolio is stored in your own student portal account and is only read or written for you when you are signed in. We do not share it, and the builder does not use AI or send your text to any outside service.

Please keep the portfolio anonymised. Do not enter patient or service user names, dates of birth, NHS or hospital numbers, addresses, postcodes or other details that could identify someone; use roles and general descriptions instead. The same applies to colleagues where their identity is not needed.

You can delete individual entries, or all of your portfolio data, at any time from within the tool. Consider exporting a copy first.

How access works

  • 12 months of access from the date your payment is completed. Buying again extends it.
  • In the student portal. The builder appears in your portal menu once you have access. If you already take our courses, it is in the same account.
  • After access ends, your portfolio stays available read-only for 90 days, so you can still view and export it. After that it is deleted.
  • Together with the workbook. If you also have the Reflective Practice Workbook, you can send reflections from the workbook straight into your portfolio. The workbook is included free with every concern pack on the course bundles page.

Many people pair the builder with the Module on Remediation, which covers root-cause analysis, building a remediation plan and evidencing change.

What the builder is, and is not

The builder is a structure and a filing system. It helps you present your own evidence clearly. It does not assess your insight, tell you what a regulator will accept, or say whether your fitness to practise is impaired. Regulators and tribunals look at the evidence as a whole, including the nature of the concern and how you have applied what you learned in practice.

A planning and writing aid, not legal advice. It does not predict any regulatory outcome. Healthcare Practise UK is an independent provider and is not affiliated with, approved or endorsed by any UK healthcare or social work regulator, the MPTS or the NHS. Please take advice from your defence organisation, union or a solicitor before you submit anything to a regulator.

Frequently asked questions

What is a remediation portfolio?

It is an organised collection of the evidence you have gathered to address a concern about your practice: courses and certificates, reflections, testimonials, changes to practice, CPD and an action plan. The builder gives that evidence a clear structure and exports it as one numbered document.

Does the builder write my reflections or summary for me?

No. There is no AI and nothing is written for you. You enter your own words; the builder organises them, tracks what is missing and formats the export.

Will my courses from other providers appear?

Courses completed on our portal appear automatically. You can add courses and other learning from any provider by hand, with the date, hours and an evidence reference.

Can I export to PDF?

Yes. The builder creates a print-ready document with a cover, contents, numbered sections and an evidence index. Use your browser's print option to save it as a PDF. A plain-text download is also available.

Who can see my portfolio?

It is stored in your own portal account and shown only to you when you are signed in. You decide who receives an exported copy.

What happens when my 12 months end?

Your portfolio becomes read-only for 90 days, so you can still view and export it, and is then deleted. Buying again before then extends your access.

Will a complete portfolio help my case succeed?

No tool can say that. A planning and writing aid, not legal advice. It does not predict any regulatory outcome. Regulators decide each case on its own facts, so take advice from your defence organisation, union or solicitor on what your evidence should include.

Not sure where to start?

Tell us your regulator and the concern. The pathway shows the courses that fit, helps you write your reflection and puts your evidence in order. It is free and nothing leaves your browser.
